Patient Portal

Welcome to Connect IVF’s patient portal.

Within the portal you will find access to your cycle specific information, consent forms, and request forms in one convenient location.

The portal is accessed using the primary patient’s details (this is the patient receiving the treatment, usually the female patient), and using the email and mobile phone details that were provided to us on your original New Patient Form. Please contact us if these details need to be updated prior to attempting to log into the portal for the first time.

Please follow the instructions below to log into the Patient Portal for the first time:

  • Please click on the link below to access the Patient Portal
  • Enter in your email
  • Click on ‘Request Temporary Password’
  • You will be asked to re-confirm your email address and answer a simple question, then click ‘Request Temporary Password’ again
  • You will then see a confirmation screen that your temporary password has been sent to your registered mobile phone
  • Once you have received your temporary password, click the ‘Log In’ button to return to the original log in page
  • Please log in using your email and newly received temporary password (please enter the password exactly as received with the dash, do not include any spaces)
  • Once you have gained access to the portal you can update your password using the ‘Change Password’ button located on the top right of the screen

You are now ready to use the patient portal to access all your sent documents per cycle, no more flipping through emails to find the document you are after!
